How It Works

CounselingFormsOnline.com is a subscription-based web site. Once you become a member (click here to subscribe) you have access to all of our forms to print out as many times as you like. Please note that you may use these forms for personal or professional use only and not for any commercial purposes.

When you view a form you can print it out right away, or move it to your Forms File Cabinet, which is where you can keep your collection of favorite forms. Just click on the Star above the form to move it to your File Cabinet.

When you want to view the forms in your Filing Cabinet, just login to the site, and click on the tab My Filing Cabinet to view the forms you have saved. Forms can be sorted alphabetically or by the date on which they were filed.
